global table prefix

tables should be {global_prefix}{plugin_prefix}{table_name}
global prefix can be set in the configs, of maybe database.php

There already is a provision for a prefix in database.php. Just need to modify the AppModel constructor to concat the global and plugin prefixes.

prefixes are pretty much removed from plugins now, should work ok if defined in the db config.
